Client Forms
Community Animal Hospital has provided our new clients with convenient downloadable forms. Download the new client form or the patient observation form, fill it out, and bring it to your appointment. We recommend that you make an appointment with us prior to coming into our facility.
Pet Medication & RX Refills
At Community Animal Hospital, we operate a full-service in-house pharmacy for your pet needs. For safe, convenient online prescription refills delivered to your door, we offer Pet Portals.
Pet Portals are private websites that give you secure online access to your pet’s health information. We provide Pet Portals free of charge to all clients who have active email addresses. NOTE: To refill your prescriptions online, you must have a Pet Portal.
Here are the three ways you can set up your Pet Portal:
- Online—Just click on Portal” button and follow instructions.
- In person—Visit our hospital and give the receptionist your email address.
- Phone—Call us on the phone and give us your email address.
Once you provide us with your email address, you’ll receive your password by email. We use your email address as your Pet Portal sign-in name. You have the option of receiving your pet’s service reminders (vaccinations, exams, and so on) by email. We also use your email address to contact clients about practice updates and important health news.
You will still be able to use your Pet Portal even if you choose not to receive emails from us. We do not sell our mailing list, and we do not disclose your personal information.
